A fiscal representative is a Netherlands-based party that handles VAT obligations on behalf of a foreign business — specifically the VAT connected to importing goods and supplying them onward. Foreign companies can't apply for the relevant import VAT deferment (known as an Article 23 licence) directly; only a Dutch entity can hold one. A fiscal representative applies for it and uses it on the importer's behalf, which is what makes the whole arrangement work.
